  • Go to New > Form
  • Fill the Form as you want to display.

Creating a form from a spreadsheet:

  • Click on the Form tab and click on Create a form.
  • In the form template that opens, you can add any questions and options you’d like.
  • Click Email this from once you’ve finished adding your questions.
  • Add email addresses for your desired recipients.
  • Click Send.
